    Case Management- Seniors Snohomish County offered at Snohomish County Human Services | Provides case management services to Snohomish County seniors and disabled persons 18 and older who require multiple services and are unable to obtain them without the assistance of a case manager.  Services include in-home assessment, development of an individualized plan of care, and follow-up to assure that clients receive the appropriate services for their needs. | Provides funding to partner agencies to contract and oversee human services.

    Transportation - Dial a Ride DART - Snohomish County

    Transportation - Dial a Ride DART - Snohomish County offered at Community Transit - Snohomish County | Provides curb to curb transportation service to ADA paratransit-eligible residents of Snohomish County.  Service is provided for customers with a disability or condition that prevents them from using regular fixed-route buses. Service is provided to those locations within 3/4 mile of local fixed routes on the same days and at the same times as those routes. | A public transportation system which provides local fixed-route bus service to Arlington, Bothell, Darrington, Edmonds, downtown Everett, Gold Bar, Granite Falls, Lake Stevens, Lynnwood, Marysville, Mill Creek, Monroe, Mountlake Terrace, Mukilteo, Oso, Quil Ceda, Snohomish, Stanwood and Sultan.  The agency provides commuter bus service to downtown Seattle and the UW District.  Community Transit operates a vanpool program and coordinates with a regional ride match system to facilitate carpools. Community Transit also provides ADA paratransit service (Dial-A-Ride-Transportation, or DART) throughout Snohomish County.
