Case Management

    Free case management for people experiencing homelessness across King, Snohomish, Pierce, and Kitsap counties. No cost to you. Funded through Medicaid.

    What We Do

    We provide Holistic Case Management to people experiencing homelessness or housing instability. Our case managers meet you where you are, in person, and help you work toward the goals you set for yourself.

    We can help with:

    • Finding and keeping housing
    • Benefits enrollment (Apple Health, SSI, HEN, ABD)
    • State ID replacement
    • Phone activation
    • Landlord coordination and housing applications
    • Employment support and job readiness
    • Connecting you with healthcare, treatment, and community resources

    Our services are funded through Washington State's Foundational Community Supports (FCS) program. There is no cost to you. We bill Medicaid directly.

    How to Get Started

    We will ask you some basic questions and check your Medicaid eligibility.

    If you qualify, we will connect you with a case manager in your area.

    Your case manager will meet with you in person to complete an assessment and start working on your goals.
